CERTIFICATE OF PROJECT COMPLETION/REQUEST FOR ACCEPTANCE AND

TURNOVER

We certify that we have completed the (Name of Contract Package) of the Sky Residence
Project inclusive of all change orders and made good all the punch lists in accordance with
the Plans and Specifications prepared BY ASYA PARTNERS & (Design Engineer) and the
Construction Agreement. We therefore request the Acceptance and Turnover of the Project.
We have submitted the following:

1. Signed and Sealed As-built Drawings

2. Test Results – if required/applicable
3. Warranty Certificates – if required/applicable
4. Operation and Maintenance Manual (1 paper copy & electronic file) – if
required/applicable
5. Treasure’s Affidavit that all labor, materials, other purchases, rentals, taxes, and other
obligations of the Contractor relative to the construction of the project have been fully paid
(Original Copy)
6. All Keys (with duplicates and tags) – if applicable
