A. Informed Consent

Dear Participant,

This survey intends to conduct an assessment on the in Guagua National

Colleges Montessori.

In view of this, the researcher requests your utmost honesty, cooperation, and
participation, by answering all the items and/or furnishing the needed information in
this questionnaire that will help in improving the operation of private schools.

The following information is provided for compliance with the research standard
of seeking informed consent from target participants:

1. The main purpose of my research is to gather the necessary data and information for
the proposal of reliable materials for an enhanced school-development program.

2. The study will cover 100 participants, of which 26 Grade 7 students, 20 Grade 8
students, 28 Grade 9 students, and 26 Grade 10 students of Guagua National Colleges
Montessori.

3. Your answers will be treated confidential and will be used solely for the very purpose
of writing this study.
