QUESTION 1

a) The term administrative controls mean measures aimed for reducing employee
exposure to hazards but not removing them. Determine 5 (FIVE) examples of
administrative control that can be applied in industrial workplace.

b) Elevator, also called lift, moves in a vertical shaft to carry passengers or freight
between the levels of a multistory building. The elevator is equipped with numerous
safety features to ensure a safe and secure operation. Explain FIVE (5) basic
elevator safety systems.

c) Noise and vibration can harm workers when they occur at high levels, or continue for
a long time. Determine what are the limit of high level noise and the risk.
Recommend FOUR (4) countermeasures for noise hazard while a workers doing
their jobs at steel industry.

d) A fire will happen when all basic elements are present at the same time. By removing
one of the elements, the fire will be able to stop. Determine these THREE (3) basic
elements and explain FOUR (4) classes of fire.

requirements. Identify five (5) hazards by explaining the possible effects and propose

QUESTION 2

a) Figure 2 shows a worker handling the welding proses. To ensure that the welding
proses activities are being carried out in a safe and healthy condition, potential
hazards need to be identified, assessed and controlled. Prepare and generate a
HIRARC report by identifying FIVE (5) hazards. Analyze the risks and suggest the
risk controls.

b) The bench-type utility grinding and buffing machine as shown in Figure 3 is suitable
for miscellaneous grinding, cleaning, and buffing works. As an engineer in the
Company A, propose five (5) safety features to minimize the risks to the employees.

a) Interpret 5 basic stages in an accident / incident investigation by stating the activities

involve in each stage. You can give an example of accident if necessary.

explain the factors together with examples.

a) Thailand Floods in 2011 affected major global corporations from Apple to Toyota to
Intel as well as local operations. If you an engineer at a particular company, clarify
four (4) recovering process to be taken before operation can be start as usual.

b) In the Implementation and Operation stage of an OSH Management System,

emergency preparedness and response are heavily emphasized. This include writing
up a Fire Drill Safety Rules. Prepare a relevant fire drill safety rules for the users of
