HEM – Change Release Note

Hospital Login

1. Upgrade Application - Upgrade Manpower

Hospitals will be able to add new manpower.

Once approved by SHA, details get added in HEM.

 Login to HEM application using Hosp ID and password


 Under "Upgrade Application" select "Upgrade Manpower"
 Fill in the required details and submit the enhancement request for approval of District/State
 You can search for enhancement request in "Enhancement Search" to check the status

2. Upgrade Application - Upgrade Licenses and certifications

Hospitals will be able to add new licenses and certifications.

Once approved by SHA, details gets added in HEM.

 Login to HEM application using Hosp ID and password


 Under "Upgrade Application" select "Upgrade Licenses and Certification"
 Fill in the required details and submit the enhancement request for approval of District/State
 You can search for enhancement request in "Enhancement Search" to check the status

3. Attachment for NABH certification

An option to upload attachment for NABH certification is provided in the portal. Refer to attachment in
the email for quick reference.

4. New Fields and fields made mandatory in the application form

Following fields are made mandatory in the application form.

 Latitude
 Longitude
 Authority registered with (New Field)
 Contact Person name is replaced with “PMJAY- Nodal Officer Name”
 Contact Person Phone number is replaced with “PMJAY-Nodal Officer Number”
 Landline number is replaced with “Hospital Admission Desk Landline no.”
 Name of the Bank account of hospital is replaced with “Name as appearing in Bank Account”

Hospital Login

1. Upgrade Application (Part 2)

Hospitals will be able to add specialties, Civil infra, medical infra and manpower details on HEM portal
and submit for approval (District /State - as per the workflow that State is following).

Once approved by SHA, hospitals gets empaneled for the applied specialty.

 Login to HEM application using Hosp ID and password


 Under "Upgrade Application" select "Upgrade Specialty"
 Fill in the required details and submit the enhancement request for approval of District/State
 You can search for enhancement request in "Enhancement Search" to check the status

District/State Login

All the enhancement requests appear under “Upgrade Specialty Worklist” tab, from where State and
District can take required action – approve or reject.

Hospital Login

1. Application Status

Hospitals will be able to track the status of application. Given below are the description for various
status used:

1 In-Draft When the hospital account is created and application is in progress
2 Submitted When the hospital submits the application
3 Withdraw When the application is not yet picked up by District for verification,
Application hospital may withdraw the form and make required changes and re-
submit the application. Please note: Already filled information stays
intact in case of application withdrawal.
4 Under District When District picks up the application and start the verification

5 Clarification When District sends back the application to hospital for clarifications.
Required from Hospital should revert within 48 hours, failing which, application is
hospital recommended for rejection to State.

6 Pending State When District transfers the application to State recommending

2. Update Application

A feature to update the existing information in the application form is provided. Hospitals will be able to
edit the “Basic Hospital Information”. This feature is enabled for approved applications. Updating such
information would not require any approval from State or District.
3. Upgrade Application (Part 1)

Hospitals will be able to edit financial information and submit for the approval of District/ State. This
feature is enabled for approved applications.
Any changes or additions made using upgrade functionality will require approval from District/State.

Note: Application Upgrade Part 2 will include adding specialties, manpower and infrastructure.

4. Inactive Applications

Application that is in-draft and not accessed by hospital for more than 30 days automatically gets inactive.
Once the hospital re-login to check the application, an OTP will be sent to the registered mobile number
and application is reverted to in-draft stage.

5. Incentive Details

In “Basic information” tab of the application, if the hospital selects the accreditation as NABH, a sub
category descripting the incentives is displayed. This is to provide the hospital with the details on
incentives based on the accreditation.

District Login

1. Initiating District Verification

District needs to click on “Under District Verification” button provided in worklist to initiate the
verification for a hospital.
