1.On or about 9:00am of September 18, 2019, Trainee CATHERINE B. AYOM was
seen and observed to be very slow in all her activities/movements in all our training
activities.

2. Initial interview was conducted and revealed that she had just delivered birth to her
baby a few months ago. Upon learning the situation, she was brought to MMDA Clinic to
check her medical status.

3. Dra. Ombina advised her to refrain from strenuous activity and certified that she is
not fit for training.
