FDAR Charting: Day 3

F > Readiness for Enhanced Comfort
D > Received patient awake on bed, “Nababawasan
ang sakit sa sugat ko” as verbalized by the patient.
Patient is able to ambulate. Able to perform ADL’s
alone. Patient is participating in nursing care done.
Patient is calm and conversant. Patient verbalize no
signs of bleeding over operating site. No Grimacing.
Patient is able to drink adequately. Able to rest during
the shift and not irritable.
A > Vital Signs monitored. Assessed pin status of
patient. Encouraged increase fluid intake and high fiber
diet. Instructed on home medication. Instructed to
come back for follow up check-up.
R > Seen by Dr. Depaynos with may go-home orders
and home medications.
