I, LAURENCE JOHN SORTIGOSA, of legal age, Filipino, Single, and a

resident of No. 5, First Street, Brgy. Our Lady of Lourdes, Jaro, Iloilo City,
after having been duly sworn to in accordance with law, hereby depose and say:

1. That I am the plaintiff in the above-entitled case and have caused this
complaint to be prepared;
