B1.2.1 All documents in Part B are to be completed and submitted as part of the tender. Failure to do so may

B1.2.2 All schedules MUST be fully completed including rates for all items listed. Tenders which fail to provide
all the required information may be treated as non conforming (hashes, “TBA” or any other expression
is not acceptable).
B1.2.3 All supporting Tender documents must be scanned for submission, including but not limited to, supporting
brochures, annual reports, insurance certificates, OH&S, AS/NZS4801 certification, Quality ISO9001
certification, similar documents etc.
B1.2.4 Tenderers will be considered to be “fully informed”. All the documentation MUST be read in its entirety.
All rates submitted (including subcontractors) will be deemed to have been prepared in accordance with all
documentation.
B1.3.1 If the tender is submitted on any Conditions of Tender which differ from Council’s Conditions, such
difference must be clearly stated in Schedule 6B. Council may treat any such tender as constituting a
non conforming tender.
B1.4.1 If the Tenderer requires any alteration to the Standard Conditions of Contract, such alterations must be
clearly stated in Schedule 6C if such a schedule is provided in these documents. Council may treat any
requirement for alteration as constituting a non conforming tender.

specified in Clause 160.A1 of the Specification shall be attached to and form part of this Schedule.
(d) In addition the Quality System descriptions for activities listed below may be requested during the Tender
consideration period:
A preliminary Construction Program shall be submitted with the Tender based on an arbitrary commencement date of
21/10/2019 that can be modified in future to a specific date at the date of letter of acceptance. Amendments may be
made prior to commencement subject to the approval of the Superintendent.
The Construction Program shall include sufficient detail and be in a form necessary to show the following:
(c) Dates for provision of approvals, delivery of materials to the site, and removal of plant and equipment to and
from site; and
(d) Milestones which identify significant events including completion of separable parts.
2. Estimated time to complete inclusive of adverse weather days allocated in Clause 100.E2 of the specification.
(Tenderers should note the date for Practical Completion specified in the General Conditions of Contract
AS2124 Annexure together with applicable Bonuses and Liquidated Damages).
The Final Construction Program will be included in the Contract, supplied and fully detailed including “critical
path” in accordance with the provisions of Clause 100.E1 of the Specification.
The Construction Program shall be attached to and form part of this Schedule.
The Tenderer is to advise the nature and extent of work proposed to be sub-contracted. The names of proposed sub-
contractors and the type of work to be undertaken are to be listed in the table below:
If the Tender is subsequently accepted by GLEN EIRA CITY COUNCIL, such acceptance will not constitute
approval by the Superintendent pursuant to sub-clause 9.2 of the General Conditions of Contract.

The OH&S Management Questionnaire is required to be completed by Tenderers and submitted to GLEN EIRA CITY
COUNCIL at the same time as (and in consideration with) their Quotation or Tender. Additional information required
under Section B of the document for OH&S. Contractor Requirements contained in the specification should also be
attached and submitted.
The objective of the Questionnaire is to provide an overview of the Tenderers OH&S Management System and
performance record. The successful Tenderer will be required to verify their responses noted in this questionnaire
through their OH&S Policy and Site Safety Plan (refer to Schedule 25A & 25C).

3 OH&S TRAINING
3.1 Describe how health and safety training is conducted in your company.
Health and Safety training and awareness is maintained through the use of Toolbox
meetings, site specific inductions, external training and site inspections. The results
derived from these practices are then discussed in the weekly employee meetings held
every Friday as well as at Management Review meetings. Please see attached Doc 15.

The information provided in this questionnaire is an accurate summary of the company’s Occupational Health and
Safety System.
Tenderers must provide evidence of their ability and capacity to comply with the requirements of the Occupational
Health and Safety Act (2004) and the Occupational Health and Safety Regulations 2007.
As a means of demonstrating compliance, the Tenderer is required to complete Schedule 25B and must provide a Site
Safety Plan.
NOTE: Prior to commencing work the successful Tenderer will be required to submit a Site Safety Plan specific
to the Contract and which as a minimum addresses those matters included in Section 111 of the Specification.
To assist in the preparing the Site Safety Plan, the Contractor is referred to the Work Safe document
“A Handbook for the Construction Regulations – Working safely in the general construction industry”.
The Site Safety Plan specific to this Contract shall be submitted within seven (7) days of a written request to do so.
