B1.1 COPIES TO BE SUBMITTED
B1.1.1 Tenders MUST be uploaded into Council’s eTendering Portal located at the hyperlink:

B1.2 DOCUMENTS TO BE COMPLETED AND SUBMITTED
B1.2.1 All documents in Part B are to be completed and submitted as part of the tender. Failure to do so may
result in Council treating any such tender as constituting a non conforming tender.

B1.2.2 All schedules MUST be fully completed including rates for all items listed. Tenders which fail to provide
all the required information may be treated as non conforming (hashes, “TBA” or any other expression
is not acceptable).
B1.2.3 All supporting Tender documents must be scanned for submission, including but not limited to, supporting
brochures, annual reports, insurance certificates, OH&S, AS/NZS4801 certification, Quality ISO9001
certification, similar documents etc.
B1.2.4 Tenderers will be considered to be “fully informed”. All the documentation MUST be read in its entirety.
All rates submitted (including subcontractors) will be deemed to have been prepared in accordance with all
documentation.
B1.3 VARIATION OF CONDITIONS OF TENDER
B1.3.1 If the tender is submitted on any Conditions of Tender which differ from Council’s Conditions, such
difference must be clearly stated in Schedule 6B. Council may treat any such tender as constituting a
non conforming tender.
B1.4 VARIATION OF CONDITIONS OF CONTRACT
B1.4.1 If the Tenderer requires any alteration to the Standard Conditions of Contract, such alterations must be
clearly stated in Schedule 6C if such a schedule is provided in these documents. Council may treat any
requirement for alteration as constituting a non conforming tender.

SCHEDULE 14 CONSTRUCTION PROGRAM
A preliminary Construction Program shall be submitted with the Tender based on an arbitrary commencement date of
21/10/2019 that can be modified in future to a specific date at the date of letter of acceptance. Amendments may be
made prior to commencement subject to the approval of the Superintendent.
The Construction Program shall include sufficient detail and be in a form necessary to show the following:
(a) Duration of each Trade activity in working days;
(b) Sequence of independent activities;
(c) Dates for provision of approvals, delivery of materials to the site, and removal of plant and equipment to and
from site; and
(d) Milestones which identify significant events including completion of separable parts.
(e) Critical Paths

3 OH&S TRAINING
3.1 Describe how health and safety training is conducted in your company.
Health and Safety training and awareness is maintained through the use of Toolbox
meetings, site specific inductions, external training and site inspections. The results
derived from these practices are then discussed in the weekly employee meetings held
every Friday as well as at Management Review meetings. Please see attached Doc 15.
Toolbox Meeting, Doc 16. Site Inspection, Doc 17. Induction Sign off, and Doc 18.
Training Record are attached.
3.2 Is a record maintained of all training and induction programs undertaken for Y
employees in your company? If YES, provide examples of safety training records.
All training and induction programs are recorded and maintained in both the
employee file and training registers. Please see attached Doc 18. Training Record
4 HEALTH AND SAFETY WORKPLACE INSEPCTION

4.1 Are regular health and safety inspections at work sites undertaken? Y
AWS Services conduct health and safety inspections for all aspects of operations
including pre-start checklists, site inspections and head office inspections. An example
of a health and safety inspection regularly undertaken on site includes Take 5’s which
are completed for every project our technicians attend. Please see attached Doc 13.
Take 5. Site inspections are completed by the compliance manager on a monthly basis
using a checklist which identifies all safety, environmental and quality aspects.
4.2 Are standard workplace inspection checklists used to conduct health and safety Y
inspections? If YES, provide details or examples.
Inspections are completed on a monthly basis and head office inspections completed
on a semi-annually basis. Please see attached Doc 21. Site inspection checklist
example.
4.3 Are there procedures for maintaining, inspecting and assessing the hazards of plant Y
operated/owned by the company? If YES, provide/attach examples.
Our procedures in place for maintaining and inspecting plant operated equipment is
to conduct pre-start checklists and ensuring the equipment is tested and tagged before
every use. Please see reference Doc 10. Equipment Maintenance and Measuring and
Doc 11. Forklift Pre-Start Checklist.

5 HEALTH AND SAFETY CONSULTATION

5.1 Is there a workplace health and safety committee? Y
AWS Services conduct weekly meetings with all levels of staff where OH&S matters
are consistently discussed with relation to past and current workplace incident
statistics and concerns. The OH&S committee is made up of the director, state
manager, compliance manager and Human resources who are involved in discussing
Health and safety practices in the workplace and coordinating HS&E processes.
5.2 Are employees involved in decision making over OH&S issues? Y
Please see attached Doc 15. Toolbox Meeting and Doc 13. Take 5. These forms have
been integrated into our CRM Software and are easily accessible to all staff and are
mandatory to be completed on all projects. Employees are required to complete the
Take 5 form each day for each project and are able to identify any OHS issues on site.
General toolbox meetings are held in head office with all staff in order to include staff
into OHS&E decision making as well as regular site toolbox meetings.
5.3 Are there employee elected health and safety representatives? Y
Comments: Each site is allocated a site representative to be in charge of the
management of the project as well as the safety aspect of the job. The Human
Resource Manager and Compliance Manager are also responsible to ensure all levels
of staff are complying to procedures and assist with OHS matters. The project
manager foresees all aspects of the work to ensure all operations are coming together
to complete the project to the highest capability possible.
6 OH&S PERFORMANCE MONITORING

6.1 Is there a system for recording and analysing health and safety performance statistics? Y
The performance of the company against health and safety standards is consistently
measured using Doc 21. Site Inspection Checklist results, Doc 13. Take 5 checklist, and
Doc 9. Incident-Accident Investigation Reports. This allows our OHS representatives
to analyse the results from each aspect and determine where the company is faulting
and the areas which need to be improved in order to prevent any hazards or
incidents occurring.
6.2 Are employees regularly provided with information on company health and safety Y
performance? If YES, provide/attach details.
On a weekly and monthly basis, information from any Doc 13. Take 5 forms, Doc 21.
Site Inspections and Doc 9. Incident reporting forms are compared in order to


accurately depict the performance of the company’s operations specifically on
OHS&E matters. Our CRM software demonstrates the frequency of an
incident/hazard occurring and the hazards/impacts that have occurred most
frequently in order to prevent any future occurrences. Employees are provided with
this information in regular toolbox meetings and management review meeting
outputs.
6.3 Has the company ever been convicted of an occupational health and safety offence? N

SCHEDULE 25C OCCUPATIONAL HEALTH AND SAFETY SITE

SAFETY PLANS
Tenderers must provide evidence of their ability and capacity to comply with the requirements of the Occupational
Health and Safety Act (2004) and the Occupational Health and Safety Regulations 2007.
As a means of demonstrating compliance, the Tenderer is required to complete Schedule 25B and must provide a Site
Safety Plan.
NOTE: Prior to commencing work the successful Tenderer will be required to submit a Site Safety Plan specific
to the Contract and which as a minimum addresses those matters included in Section 111 of the Specification.
To assist in the preparing the Site Safety Plan, the Contractor is referred to the Work Safe document
“A Handbook for the Construction Regulations – Working safely in the general construction industry”.
The Site Safety Plan specific to this Contract shall be submitted within seven (7) days of a written request to do so.
