JOINT-AFFIDAVIT OF TWO (2)

DISINTERESTED PERSONS
WE, DENNIS DIALANE ISTURIS and ADELE ALBISO ARSENIO, both of legal age,
Filipinos, single, and residents of Brgy. Tibal-og, Sto. Tomas, Davao del Norte, Philippines,
after having been sworn to in accordance with law, do hereby depose and say that:

1. We are the same affiant who personally and voluntarily caused the preparation
and execution of this joint-affidavit;

2. We personally know MARITESS ANTONIO BUSTOS, who was born on May 1,

1981 at Pagsabangan, Tagum City, Davao del Norte, Philippines to spouses Violeta
Antonio and Bienvenido Bustos;

3. Upon verification of her Elementary School Permanent Record and Certificate of

High School Graduation, issued by the Department of Education, it appears that
her first name was erroneously recorded as MARITES, instead of MARITESS,
which is her true and correct first name;

4. Upon verification of her Transcript of Records with two (2) pages, issued by
Monkayo Community College, Monkayo, Davao, it appears that her first name was
erroneously recorded as MARITES and MARKITES, instead of MARITESS, which
is her true and correct first name;

5. Being our long-time friend and neighbor, we know for a fact that the
aforementioned person’s true and correct name is MARITESS ANTONIO BUSTOS,
and that the names MARITES ANTONIO BUSTOS, MARKITES ANTONIO
BUSTOS and MARITESS ANTONIO BUSTOS, refers to one and the same person;

6. We are not related to the aforementioned person either by affinity or

consanguinity;

7. We are executing this affidavit to attest to the veracity of the foregoing

declarations, to apprise the authorities concerned of the aforementioned facts and
for whatever legal purpose this may serve best.

IN WITNESS WHEREOF, we have hereunto affixed our signatures this October 11,
2019 at Sto. Tomas, Davao del Norte, Philippines.
