AFFIDAVIT OF GUARDIANSHIP
I, VIRGILIO A. BRACEROS, of legal age, Filipino, married and a resident of Purok
1, Brgy. Tibal-og, Sto. Tomas, Davao del Norte, Philippines, after having been sworn to in
accordance with law, do hereby dispose and state that:

1. I am the same affiant who personally and voluntarily caused the preparation of
this affidavit;

2. I am the biological grandfather of ZHYRA BRACEROS, who was born on April 27,
2017 at Southern Philippines Medical Center, Davao City, Davao del Sur,
Philippines. She is the first (1st) offspring of my biological daughter, Dovie Angel
Saja Braceros to an unknown father;

3. It bears stressing that Dovie Angel Saja Braceros went to Qatar to work as a
Janitress for a certain company since 2017 up to the present;

4. Due to the aforementioned circumstances, I henceforth become the sole guardian

exercising substitute parental authority as well as legal guardianship to my herein
minor grandchild;

5. I am executing this affidavit to attest the veracity of the foregoing declarations, to

apprise the authorities concerned and for whatever legal purpose this may serve
best.
