AFFIDAVIT OF ONE AND THE SAME PERSON

I, Jose L. Rodriguez, Filipino, married to Pilar A. Mateo, of legal age, with

address at Rm. 3101 Makati Executive Tower 2, Dela Rosa St. Makati City after having
been duly sworn to in accordance with law hereby depose and say:
1) I have used the name JOSE L. RODRIGUEZ, in all of my records and dealings
such as Driver’s License, government IDs, school records and others of similar
nature.

2) My baptismal certificate states my name as JOSE L. RODRIGUEZ. It is the

ONLY document which is in my possession to which my identity is based. As
such, I have used the name “JOSE L. RODRIGUEZ” instead of “JOSE L.
RODRIGUEZ JR.” which is the name appearing in my Birth Certificate.

3) I am under the impression and belief that “JOSE L. RODRIGUEZ” is my real and
ONLY name;

4) It was only during the time that I need to secure requirements for my passport
that I discovered that a name “JOSE L. RODRIGUEZ JR.” appeared in the
authenticated copy of my Certificate of Live Birth instead of the name “JOSE L.
RODRIGUEZ” which I am using since childhood.

5) I am executing this affidavit to attest that “JOSE L. RODRIGUEZ” and “JOSE L.

RODRIGUEZ JR.” as appearing in my Certificate of Live Birth are ONE AND
THE SAME PERSON.

6) I am executing this affidavit to attest to the truthfulness and honesty of the

foregoing facts and for all intents and purposes it may serve.
