AFFIDAVIT OF DISCREPANCY AND EXPLANATION

I, CENEN ALARCON RUGA, of legal age, married, and with address at Brgy. Sinamongan, Pilar,
Capiz, after having been sworn in accordance with law, hereby depose and state:

1. That I am the mother of ALEXIE CEN ALARCON RUGA who was born on 7 May 1999;

2. That in his Certificate of Live Birth, her middle name appears to be entered as “ALAROON”
instead of “ALARCON”;

3. That a close examination of the said middle name clarifies the discrepancy; that the “C” before
the “O” is closely attached to the latter thereby giving the appearance that the middle name of
my child is “ALAROON” instead of “ALARCON”;

4. That the true and correct middle name of my child is “ALARCON” and not “ALAROON”;

5. That there is no reason to rectify his Certificate of Live Birth considering that a careful
examination of her middle name reveals that the same in in fact “ALARCON”;

6. That I am executing this Affidavit to explain the supposed discrepancy appearing in the
Certificate of Live Birth of my child.

TO THE TRUTH OF THE FOREGOING, I have hereunto affix my signature this ___ March
