SUBJECT : Transmittal of documents of PSMBFI platinum members
DATE : October 17, 2019

The Marketing Department transmits the application forms of 47 Platinum members

assigned in various Regions. 3 member from 1998, 5 members from 1999, 7 members
from 2000 and 32 members from 2001 . These were submitted through mailing by our
Regional Extension Offices.
This transmittal consists of the following:
a) Attached list of Platinum members with filled-out forms;
b) Computer-generated letter to the Insurance Manager;
c) Membership Application Form;
d) Authority to Deduct (MEP, BGTP, and BAB); and
e) Dependent’s Free Insurance Coverage Form.
f) PSMBFI’s Data Privacy Agreement
g) Member’s Service Record

These concerned PSMBFI members were educated of their benefits and privileges
should they shift from the Mutual Benefit Equity Plan (MBEP) into the Members Equity
Plan (MEP). They were also informed that their Equity Value will be processed after 3
months, hence enticing them to continue their PSMBFI membership.
