SUBJECT : Incident Report involving Catherine E. Trimor

This pertains to the incident that transpired last October 4, 2019 at about 11:30 a.m at
Customer Service Section. Wherein, Mrs. Delia Quila identified as the wife of PMaj
Joselito E. Quila a PSMBFI member assigned at MIMAROPA – PRO 4B visited the office
to file a loan on behalf of PMaj Quila.
Mrs. Quila approaches Chino Dimaano, Marketing Assistant – Customer Service Section
to inquire and file a loan, Upon checking some requirements are missing signature and
payslip, Chino even asked if she knew the payroll ID and password and she said “Hindi”.
Chino ask Mrs. Quila to comply the requirements, she then leave the office.
At about 12:00 p.m. same date Mrs. Quila returned to Customer Service and approaches
Catherine Trimor, presented the loan application, same requirements she presented to
Chino but with attached Special Power of Attorney dated Februaruy 11 2015. During the
process of the said loan Catherine instructed Mrs. Quila to proceed to Investment
Department (Window 15 or 16) to file the loan.
Several minutes after the instruction, Severino Mora – Loan processor approaches the
Customer Service Section, informing us about the lapses in the requirements and the
incident wherein one of the employee from Customer Service suggested Mrs. Quila to
duplicate the signature of the member, and found out that it was Catherine Trimor who
gave the instruction.
During the discussion with Ms. Michelle Cayabyab – Investment Manager, Nhel Ramos
– Investment Supervisor, Mrs. Quila and the undersigned, Catherine Trimor admitted that
she suggested Mrs. Quila to duplicate member’s signature, for the reason that PMaj
Joselito E. Quila is not available to personally file the loan because he is presently
confined in the hospital and in need of immediate assistance.
