AFFIDAVIT OF DISCREPANCY IN THE LAST NAME

I, LENNIE ANN TAER ORAP-ORAP, of legal age, single, and with address at 3452
Garcia St., Brgy. Pinagkaisahan, Makati City under oath, state that:

1. THAT my name is LENNIE ANN TAER ORAP-ORAP as stated and registered

in my birth certificate issued by office of the Civil Registry of Makati;

2. A copy of my birth certificate is hereto attached as ANNEX “A”;

3. THAT the name LENNIE ANN TAER ORAP-ORAP also appears in my passport
which was issued based on the data appearing in my birth certificate. A copy of
the page of my passport where my name and picture appear is hereto attached
as ANNEX “B”;

4. THAT from the roots of my family, my ancestry’s last name is spelled “URAP-
URAP”, however due to confusion or miscommunication by reason of
misinterpretation of penmanship or understanding by the receiver of the
information of my last name at the time the name was being registered, such
last name was inadvertently misspelled. Hence, despite the discrepancy I
managed to use the correct spelling of my last name LENNIE ANN TAER
URAP-URAP in order to be consistent with the correct and true spelling of my
lineage since my childhood. Thus since then I have been accustomed to using
the name LENNIE ANN TAER URAP-URAP in my personal records,
transactions and communications;

5. THAT the name LENNIE ANN TAER ORAP-ORAP and LENNIE ANN TAER
URAP-URAP refer to one and the same person, the herein Affiant;

6. THAT I am executing this Affidavit to attest to the truth of the foregoing facts and
to use the same for whatever legal purpose it may serve.

registered in my birth certificate issued by office of the Civil Registry of Makati;

3. THAT the name HODARICO OCCEÑO URAP-URAP also appears in my

passport which was issued based on the data appearing in my birth certificate.

communications;
