Heartwood Medical Practice is a well-established GP Practice. Its services are delivered from
Swadlincote Health Centre. The Practice premises are modern and purpose built. There are four
GP Partners and 2 salaried GPs, supported by a practice team. Our nursing team has a range of
skill mix, including Nurse Practitioners, Practice Nurses and Health Care Assistants.

We are a forward-thinking innovative practice working within the Derby and Derbyshire Clinical
Commissioning Group. The Practice is committed to providing the best service it can to all
registered patients. As judged by QOF and external inspection the Practice provides a high quality
of care and has maintained this over recent years. The Practice was inspected by the Care Quality
Commission (CQC) in 2016 and rated as good. For further information read here:

To support our growth and workforce development, we are looking to appoint one full time or two
part time, experienced, self-motivated practice nurses to join our friendly team of qualified health
professionals serving a patient list of 8300.

Candidates must have the ability to undertake general practice nursing duties such as travel
vaccinations, childhood immunisations, cervical sampling, dressings and preferably chronic
disease management.

Knowledge of SystmOne is an advantage, however full training will be given.

Skills in all of the above areas are desirable, but we will invest in training for the right applicant if
there are any skill gaps.

Job Description – Practice Nurse
Job Title: Nurse

Job Purpose: To provide a high quality care service based on the needs of the
practice population which includes treatment, screening,
preventative care and patient education

Duties and Responsibilities – Desired but not Essential


Clinical
To manage patients with chronic disease, running specific clinic sessions

Participate in national screening work such as cervical cytology and deliver

Administration
To maintain accurate patient records and enter onto the computer using

General
To maintain own annual registration with the NMC and adhere to Code of
