JOINT AFFIDAVIT OF TWO DISNTERSTED PERSONS

(FOR DELAYED REGISTRATION OF BIRTH)

We, MARY JOEY L. KEDSE and JOY A. DIATEN, Filipino Citizens,

both of legal ages, and with residences at 537 San Vicente, Baguio City and 531
San Vicente, Baguio City respectively after having duly sworn to in accordance
with law, hereby depose and say that:

1. We know the facts and circumstances surrounding the birth of thee

child named CYRHIEZ ANN GATUDAN KEO-SEN who was born
on June 21,2015 at Edmonton Alberta, Canada to her biological parents
ANDREW D. KEO-SEN and BELEN G. KEO-SEN both being
personal acquaintances and neighbors in our residence;

2. The parents of the said child were validly married on October 12, 2000
at Baguio City, Philippines and solemnized by Baguio City Mayor
Mauricio Domogan;

3. The birth of the said child was attended by Dr. Agnes Lee, a duly
licensed physician of Grey Nuns Community Hospital at Edmonton, AB,
Canada;

4. The said child was immunized but not yet baptized since one of the
requirements for baptism is the child’s birth certificate which is still
being applied for delayed registration;

5. The birth certificate of the said child was not immediately registered
with the Office of the City Civil Registrar of Baguio City, primarily
because of the inadvertence of the parents when they went home from
Canada as an OFW;

6. The birth certificate of the said child is not registered with any Office of
the Civil Registrar;

7. We are executing this Joint Affidavit to attest to the truth of the

foregoing facts of the birth and to support the application for delayed
registration of the same with the Office of the City Civil Registrar of
Baguio City;

IN WITNESS WHEREOF, I have hereunto set my hand this 5th day of

September 2019 in the City of Baguio, Philippines.
