AFFIDAVIT OF ACKNOWLEDGEMENT
AND ALLOWING THE USE OF FATHER’S SURNAME

I, MARCELO TANGAN MERLES, of legal age, married, Filipino, and a resident of Bangag,
Solana, Cagayan, and after having been sworn to in accordance with law, hereby depose and
state:

1. That I am the natural/ biological father of LALA SIMANGAN and DEXTER

SIMANGAN whose date of birth was on July 23, 1996 and November 20, 1997, respectively at
Bangag, Solana, Cagayan as evidenced by the attached Certificates of Live Birth and hereto
marked as Annexes “A” and “B”;

2. That my children was born out of my cohabitation with wife ELVIRA CALIMAG
SIMANGAN, without the benefit of marriage but without any legal impediment to marry with
each other;

3. That I hereby declare and acknowledged that I am the natural/biological father

of LALA SIMANGAN and DEXTER SIMANGAN and I hereby acknowledged that they are my
own daughter and son, and therefore entitled them all rights and privileges appurtenant
thereto as provided for by law, including to use my surname as their own father;

4. That being the father of my children, I am now giving authority to my daughter [

LALA SIMNGAN] and son [ DEXTER SIMAGAN ] to use my surname MERLES so that they can
now be called for all intent and purposes as LALA SIMANGAN MERLES and DEXTER
SIMANGAN MERLES;

5. That I am executing this Affidavit in order to attest to the truth and veracity of
the foregoing statements;

6. FURTHER AFFIANT SAYETH NAUGHT.

AFFIDAVIT OF LEGITIMATION
WE, ORLANDO FRANCISCO ORQUE and JENNIFER RALUTIN LANORIA-
ORQUE, both of legal ages, married to each other, Filipinos and residents of Liwan Sur,
Enrile, Cagayan, under oath, do hereby depose and state:

1. That we are the natural/biological parents of JOHN REY LANORIA

ORQUE, who was born on October 19, 1997 at Liwan Norte, Enrile, Cagayan
as evidenced by his Birth Certificate which is hereto attached as Annex “A”
hereof;

2. That our son JOHN REY LANORIA ORQUE was born during our
cohabitation without the benefit of marriage but without any legal
impediment to marry each other;

3. That at the time of his birth we were both of legal age;

4. That on October 22, 1997, we contracted our marriage before Hon. Robert B.
Turingan, Municipal Mayor of Enrile, Cagayan, as evidenced by the Marriage
Contract which is hereto attached as Annex “B” hereof;

5. That our son JOHN REY LANORIA ORQUE should be legitimated by

virtue of our subsequent marriage and shall enjoy all the rights and priviliges
of legitimate child and consequently use the surname of his father;

6. That we are executing this affidavit to attest to the veracity and truthfulness
of the above-statements.
