I, ROSALIE D. CALSA, Filipino, of legal age, single parent, after being sworn
according to law hereby depose and state:

That I am the de-facto guardian of JOMAR S. ARANZADO, a minor and my

nephew, whose parents have left him with us after their separation,

That JOMAR S. ARANZADO is staying with us and relies on us for his subsistence
and education.

That I am executing this Affidavit of Support to establish the fact that minor
JOMAR S. ARANZADO is under my custody as his de facto guardian.
