Fall Through Deck Opening

On Saturday, June 1, 2019, a wireline contractor suffered fatal injuries
due to a fall on Chevron’s Genesis facility. Our investigation is ongoing,
but information gathered immediately after the event suggests the Date Event Occurred:
following: June 1, 2019
A wireline crew had finished work on well A-14 and was starting work on
well A-4. The deck opening over well A-14 was properly hard-
barricaded, but the hatch cover had not yet been replaced. Two workers Location:
went to an adjacent well bay hatch cover, over the A-13 well slot, and

lifted that hatch cover. This action unintentionally created an open hole,

and one worker stepped into the open hole, resulting in a fatal fall.

The correct well bay hatch cover for the A-14 well was located off the
well bay adjacent to the A-14 well slot.

Local alarms were sounded, and emergency medical responders
General Manager - HES
immediately responded. All other work was immediately suspended and
jaime.carbajal@chevron.com
internal and external agency notifications were made. The investigations
are ongoing, and plans will be made to safely re-start work when
appropriate.
