Declaration for travelling to

Australia

I, undersigned Gioga Cristian, living in Romania, Constanta, on the

street Dezrobirii, nr 82A, Etaj1, Apartment3, I am requesting the visa in
behalf of my employer Holland America to join one of their vessels.
I don’t know for the moment which date I will be in Australia, I am just
requesting this visa for the near future when will be needed.
On board of the vessel I will have the rank as Electrical Officer.
