PARENTS’ CONSENT

To Whom It May Concern:

given my full consent to allow my son/daughter to participate in the

the development of my son/daughter and that I am willing to provide financial assistance. As,
parent I have reminded my son/daughter to exercise extra care in the performance of the said
activity. Furthermore, I have reminded my son/daughter to obey policies and regulations
imposed by the school administration relative to the said activity.

CONDITIONS FOR ACTIVITIES OUTSIDE THE CAMPUS

1. That the organizer/advisers shall strictly impose safety and security

measures and policies to ensure smooth, orderly and successful activity.
The participant/student hereby agrees to faithfully abide by these.

2. That should any harm or injury befell upon a participant/student by

reason of his/her own negligence, omission or violation of the rules
implemented, the organizers/advisers shall not be held liable for any
resulting damage/s, whether this takes place within the confines of the
venue or outside the stated venue/function;

3. That in case of any accident, act of God or force Majeure including

but not limited to civil commotion/disobedience and acts of public
enemy’s; the organizers shall not be liable for any loss, damage or injury
by reason thereof;

4. That should a participant/student decide to go out of the itinerary

to explore the place, to visit a relative or friend, to attend a gimmick.
He/she must look after his/her own welfare. The participants/student must
inform the adviser of the organizers of his/her intended destination.

5. That the organizers/advisers reserve the right to disallow a

participant/student to continue attending the activity in the event of any
violation of the security and safety measures.

6. That the participant/student hereby declares that he/she read and

understood the forgoing conditions and that he/she undertakes to abide
by the same.
