KNOW ALL MEN BY THESE PRESENTS:

We, MELECIA C. PORPAYAS, of legal age, Filipino citizen and resident of Brgy. Sta.
Cruz, General Luna, Surigao del Norte, Philippines, ALDIGONDA C. GORGONIO of legal
age, Filipino citizen and resident of Brgy. Libertad, General Luna, Surigao del Norte,
Philippines, EXCELENCIA M. GORGONIO of legal age, Filipino citizen and resident of Brgy.
Libertad , General Luna, Surigao del Norte, Philippines, BERNANDITA C. BLANCADA of
legal age, Filipino citizen and resident of Purok 3, Brgy. Sta. Fe, General Luna, Surigao
del Norte, Philippines, and MELECIANO M. CAMINGUE of legal age, Filipino citizen and
resident of Katipunan, Pilar, Surigao del Norte, Philippines, after having been sworn to in
accordance with law, do hereby depose and state:

1. That the above-named parties are the only Children and Compulsory Heirs of
CONSESA MAZO;

2. That the decedent died intestate, without leaving any Last Will or Testament and
without leaving any debts or accounts whatsoever;

3. That the deceased left a certain parcel of lands located in Katipunan, Pilar,
Suirgao del Norte and more particularly described as follows:

“A parcel of land situated at Katipunan, Pilar, Surigao del

Norte, Philippines with Lot No. 7399, Tax Declaration No. 13-
16-0010-0047, with the following boundaries: North: Lot No.
7332; East: Lot No. 7398-RIVER; South: Lot Nos. 7400-7401-7402;
and on the West: Lot Nos. 7404-7331; containing an area of
more or less 9.6762 has., declared in the name of the late
Consesa Mazo”

4. That pursuant to Section 1 of Rule 74 of the Revised Rules of Court of the

Philippines, and both being with full capacity to contract, we do hereby
adjudicate unto ourselves the land described above, in equal shares pro indiviso;

5. That we hereby affirm that we have executed this instrument out of our own free
will without force, intimidation or violence upon our person, and that we have
hereby received our just and proper share and have no more claim or demand
against each other in connection with the adjudication of the above-described
parcel of land;

6. That, we, MELECIA C. PORPAYAS, BERNANDITA C. BLANCADA, EXCELENCIA M.

GORGONIO, ALDIGONDA C. GORGONIO and MELECIANO M. CAMINGUE, hereby
TRANSFER and CONVEY all our rights, interest and participation by way of WAIVER
on certain PORTIONS of the above-described parcel of land for being SHARES of
the owner to whom they are waived at. It is further described with the following
technical descriptions:

“A parcel of land situated at Katipunan, Pilar, Surigao del

Norte Philippines, with Lot No. 7399-P, Tax Declaration No.
2017-11-0010-00296 PIN No. 048-11-0010-007-05, with the
following boundaries: North: Lot No. 7332; East: Lot No. 7399-
P; South: Lot No. 7400; West: Lot No. 7399-P; with an area of
 more or less Six Thousand One Hundred Seventy-Three Square
Meters, under the name of Consesa Mazo”
