Patient came to the clinic for a physical.   Patient’s health is fine.  There are no pathologies 
or abnormalities. 

While she was here we discussed the birth control implant that she had had installed 17 
months earlier, and she asked to remove it, alleging that she had forgotten about it.  She 
explained that she and her husband had decided to try to have a child, and she wanted to 
know whether there would be any delay in being able to conceive once the implant was 
removed.  I explained to her that there would be no delay. 

Upon her confirming her request, I removed the implant.
