AFFIDAVIT OF DESISTANCE AND REQUEST FOR THE

RELEASE OF DRIVER’S LICENSE

I, MARIO PANCHO, of legal age, Filipino, and a resident of Oslob, Cebu after having
been duly sworn according to law, hereby depose and state that:

1. I am the father of John Peter Benologa Pancho a victim of truck sideswept on May
17, 2013;

2. We have already settled the matter amicably and the company, Cebuana Logistica,
Inc. whom the truck is assigned to paid all the medical expenses & hospitalization of
my son, John Peter B. Pancho.

3. I execute this affidavit to attest to the truth of the foregoing statements and for
whatever legal purpose this may serve; this is likewise executed with the request for
the release of the driver’s license of the driver, Danilo Catigan on hold with Oslob
Police Station.
