AFFIDAVIT OF LOSS
I, JOSE VILLAMOR TORRES SAN PASCUAL, single, of legal
age, with address at 49B San Rafael St., Brgy. Plainview, Mandaluyong City,
under oath, depose and state that:

(1) I am the son of ROSALIA TORRES SAN PASCUAL, the

registered owner of a house and lot, measuring one hundred sixty
nine (169) square meters, located at Brgy. Zaniga, Mandaluyong
City.
(2) This property is covered by Transfer Certificate of Title (TCT)
No. 0896, registered under her name.

(3) I can no longer find the Owner’s Duplicate Certificate of Title

until today. I have exerted utmost diligent efforts to locate it, but
unfortunately, my search efforts were all in vain. In my honest
belief, the same was already lost.

(4) I am executing this affidavit to attest to the truth of the foregoing

and to use as basis to file a petition for the issuance of a new
Owner’s Duplicate Certificate of Title and for other legal
purposes.
