AFFIDAVIT OF RENUNCIATION OF INHERITANCE

KNOW ALL MEN BY THESE PRESENTS:

I, CDE, Canadian, of legal age, and a resident of

_________________________________, Canada, after having been duly sworn
according to law, hereby state under oath that:—

1. My unmarried uncle, JDE died on ____________________ in Manila. He left

no descendants, ascendants, illegitimate children, single brothers and sisters. I am
attaching hereto as Annex “A”, a copy of the decedent’s Death Certificate.

2. I and my siblings are the only remaining collateral relatives of decedent,

being the children of his deceased brother MDE.

3. To prove their fraternal relationship of decedent JDE and our father MDE, I
am attaching hereto as Annexes “B” and “C”, respectively, the birth certificate of JDE and
the marriage contract of our father MDE, both indicating the identity of their common
parents, who are ALE and BMF.

4. To show my filiation to MDE, I am attaching hereto as Annex “D”, a copy of

my birth certificate naming MDE as my father, thus, making me the nephew of the
decedent JDE.

5. As a surviving children of JDE’s deceased brother MDE, I and my siblings are

called to succession in the Estate of JDE by right of representation of my deceased father
MDE by virtue of Article 972 of the Civil Code of the Philippines.

6. Decedent, at the time of his death, was the registered owner of a parcel of
land with improvements located in Quezon City and described therein as:—

“xxxxxxxxx copy technical description of property xxxxxxxxxxx”

6.1. I am attaching hereto as Annexes “E”, “F” and “G”, copies of the land
title and Tax Declarations of the foregoing properties.

7. However, I am not interested in such inheritance. Therefore, I, CDE, hereby

renounce, abandon, and relinquish all rights, titles, and interests in and to any gift,
inheritance, bequest, or other property or asset of the Estate of JDE.
