DEPOSITION OF WITNESS
and SURVEILLANCE
THE UNDERSIGNED, legal age, married, and a member of the Philippine
National Police, presently assigned at the Tabaco Municipal Police Station Tabaco,
Albay and, designated as Intel- Operative, UNDER OATH, hereby states THAT:

1. In the month of August 2017, a series of report from my confidential informant

revealed that the respondent who is the subject of the search warrant, is a
personally engaged in the selling and use of illegal drugs specifically "Cocaine"
 or "Shabu" and his using [respondent] house as the place of disturbution and
use.
2. I and my confidential informant conducted the undercover surveillance upon
the respondent for several weeks and we had arranged a buy-bust operation
but the respondent seems to be cautious enough which spoiled the said
operation;
3. For the continued monitoring upon the subject there are male persons on
board different vehicles who comes in and out of the respondent's house at
anytime of the day and night. On september 10, 2017 at about 2:15 PM I and
my confidential informant had conducted a test buy against the subject at his
residence;
4. We were on board my vehicle when we drove to Brgy. Baranghawon [Tabaco]
and alighted at the house of respondent. My informant called the respondent
"TOK", "TOK" "BRAD" and knocked on the door of the house. The respodent
replied "OH SISAY AN?" [WHOSE THAT?] my confidential informant replied his
nickname [to be withheld for security purposes]. The respondent opened the
door and asked "ANO TABI BRAD?" [WHAT IS IT BRO?]. My CI replied " MA
SCORE LAMANG KMI BRAD KI LIMA JURUJAMMING TURO-TAD TAOD" [WE
WILL SCORE FOR FIVE FOR OUR JAM LATER];
5. My CI asked from P500.0 and I handed the money to respondent and then the
respondent came back and gave to me a transparent plastic sachet of
suspected Shabu. Afterwards, we said goodbye to the respondent and told him
that we will return soon.
6. I left my CI at Tabaco. and I provided to. At his office to report the
successful test buy operation. At said office, instructed me to put my name
initials on the said plastic sachet of suspected Shabu I bought from the subject
and then submitted the same at the Regional Crime Laboratory 5 in Legazpi
City where it yielded positive of Shabu, an illegal drug.
7. I attest to the truthfulness of the above statements and shall give other details
in Court when told to do so.
