Answer the following questions in 150 words or more.

1. Which techniques in Root Cause Analysis would you prefer to use? Why?
Among the seven techniques under root cause analysis, the one I prefer the most is
Fishbone diagram also known as Ishikawa or cause-effect diagram. I believe that this is
most fitting for me because it provides a more organised way to analyse problems. In this
diagram, the causes and sub-causes of a problem are being grouped according to their
categories. This diagramming technique makes it easier to group causes together which
may be under the 4Ms (manufacturing), 4Ss (service) or the 8Ps (also services)
depending on the industry. Fishbone diagram is very useful in preventing defects in the
product design and quality of a system. The reason why I prefer this over the other root
cause analysis is because it makes analysing problems simpler, I also like how it does not
only focus on one possible source of a problem and it is able to take down all causes or
sub-causes of a problem in an organised manner.

2. Why do we need to maintain the quality of data? What are the benefits of using
data quality tools in the hospital setting?

Data quality is the use of datasets as a function, it is determined by its ability to be


analysed for a database, data warehouse or data analytic systems and how efficient it is
in processing information. It is important to maintain quality data because it affects all the
processes done in a health care facility, and one mistake may lead to various errors. Data
must be accurate, complete, relevant, consistent, reliable, presentable and accessible,
these aspects are to always be considered for data quality. Without data quality, data will
not be considered useful or consistent. Data quality tools play an important role in health
care systems because it determines whether or not the data contains no error and that it
is reliable to be used for the different processes in a health care facility. Benefits brought
by data quality tools include increase in hospital revenues, reduced costs that may come
with side effects or erroneous data, reconciling data will require less time, greater
confidence in the system and last but not the least, increased patient satisfaction.
