III. PROBLEM PRESENTED


The subject patient was admitted on August 25, 2018 accompanied by his wife and escorted
by the police for taking excessive illegal drugs (Shabu) and being addicted in alcohol.

IV. BRIEF HISTORY OF PRESENTED CONFINEMENT


The patient has a realization in his staying in the rehabilitation center. He realizes the
importance of family and his wrong actions in his life. According to him, “I easily adopted the
changes of the environment because it is my 6th time to be in the rehabilitation center but it is
my 1st time here in CLDRC (Central Luzon Drugs Rehabilitation Center)”. He felt new to the
programs of said rehabilitation center and he learned to be friendly. He also added, he is now
more socialized than before and as of now he didn’t commit any violation from the rules of the
rehabilitation center from the day of his admission.

V. SUBSTANCE ABUSE USE HISTORY AND TREATMENT


The subject patient started to take illegal drugs on 1994 at the age of 17 at Guadalupe, Makati
City with his friend because of curiosity. He continued taking drugs that led him in addiction.
He bought shabu at the price of 300 Pesos. He took shabu five (5) times a day and it affected
many aspects of his life. At first it gave strength to him physically but after a months of taking
illegal drugs he found himself physically declined and having unnecessary gestures. It affected
his relationship to his family that caused more misunderstanding between his wife and children.
He socialized with limited people because they knew that he’s taking illegal drugs and forget
to attend church services because he felt so ashamed of what he is doing. His last intake of

illegal drugs was on August 24, 2018 the day before he entered rehabilitation center and the
drug test’s result indicate that he is positive in using illegal drugs.

VI. PSYCHIATRIC HISTORY


The subject patient has no any psychiatric history but the current rehabilitation center
recommended that he need an assistant from the psychiatrist due to his anxiety.

VII. FAMILY AND SOCIAL HISTORY


The subject patient lives in Antipolo City but has an ancestral house in Cainta, Rizal. He has
7 siblings and all raised by their parents. His father was a sales man in Toyota and his mother
was a house wife. As of now he is married with Rizza and has a 2 children. His wife is a call
center agent and his daughter is at the age of 12 and His son is at the age of 16. The family of
the subject patient has no any involvement in criminal liabilities. In his staying in the
rehabilitation center he was supported by his wife and siblings financially and to his basic
needs.

VIII. EMPLOYMENT HISTORY


The subject patient worked at Dairy Queen at the age of 16 as a service crew. His salary was
100 per day. Year 2000 he worked at GMA Network (News Public Affair) as an assistant
camera man until 2002 because he entered a rehabilitation center but he worked again in the
same network and position on the year 2004 to 2014. His last work in GMA Network was a
visual effect editor in the year 2015. One of the shows that he worked was the Celebrity Bluff.
His salary per shoot was 2, 500 pesos.

IX. MEDICAL HISTORY


The subject patient has no any current medical problem but at the age of 37 he was diagnosed
of having fatty liver because of excessive drinking of alcohol. This led him to be physically
weak.

The subject patient has no any legal history.

XI. MENTAL STATUS EXAMINATION


The subject patient was comfortably sharing his story. While he was talking, I observed that
he can look into my eyes, his gestures was normal but his physical appearance was not yet
fully-recovered in effects of illegal drugs that he had taken
