Coronary artery
หลอดเลือด coronary เป็นหลอดเลือดที่วางตัวอยู่บริเวณชั้น epicardium ของหัวใจ มีหน้าที่นาพาเลือดและ
สารอาหารต่างๆไปเลี้ยงกล้ามเนือ้ หัวใจ โดยการทีเ่ ลือดจะไปเลีย้ งกล้ามเนื้อหัวใจได้นั้น ต้องอาศัยความต่างระหว่างความ
ดันในหลอดเลือดcoronary และความดันของกล้ามเนื้อหัวใจ(ventricular pressure) เรียกว่า coronary perfusion
pressure
เป็นที่ทราบกันดีอยู่แล้วว่าแรงดันในขณะที่กล้ามเนื้อหัวใจบีบตัว(ventricular systole) มีค่าสูงมากเกินกว่าที่
ความดันของหลอดเลือดcoronaryจะชนะได้ ดังนั้นกล้ามเนื้อหัวใจจึงเป็นอวัยวะชนิดเดียวของร่างกายที่รับเลือดแดงใน
ขณะที่หัวใจคลายตัว (diastolic phase)
ในคนที่ไม่มีความผิดปกติของหลอดเลือด coronary แรงดันในหลอดเลือด coronary จะเท่ากับ aortic diastolic
pressure เนื่องจากเส้นเลือดนี้เป็นแขนงโดยตรงที่ออกจาก aortic root บริเวณที่เรียกว่า sinus of valsava โดยมี2 แขนง
ใหญ่ คือ Rt.coronary atery (RCA) และ Lt.coronary artery (LCA) ดังรูป
กล่าวได้ว่า
Coronary perfusion pressure = aortic diastolic pressure - left ventricular end-diastolic pressure (LVEDP)
LAD= Left anterior descending a. , CX=circumflex a. , S=septal perforator , D=diagonal a. , OM= obtuse
marginal branch , RCA=Rt.coronary a. , AcM=Acute marginal branch , PD= posterior descending a. ,
PL=Posterolateral a. , SN=branch to SA node

Clinical presentation
1) angina pectoris
2) positive stress test
3) acute MI
4) sudden death with resuscitation
5) cardiomegaly and symptoms of chronic heart failure without any other obvious cause.
Diagnostic Tools
นอกเหนือการตรวจวินิจฉัยโรคกล้ามเนื้อหัวใจตายเฉียบพลันเบื้องต้นได้แก่ EKG, CXR, cardiac enzyme
แล้ว การตรวจเพิม่ เติมเพื่อดู anatomy และ myocardial function ก็มีความจาเป็น โดยเฉพาะในกรณีที่จะทาการรักษา
โดยวิธrี evascularization
Diagnostic Imaging of coronary artery disease (anatomical consideration)
Non-invasive angiographic technique
1) Multidetector computed tomography coronary angiography (MDCT) เป็นการประเมินเพียง imaging
ไม่สามารถบอกถึง hemodynamic significantได้ มีค่า negative predictive value(NPV) ค่อนข้างสูง ประมาณ 83-89%
แต่มีคา่ positive predictive value ปานกลาง มักให้การแปลผลที่ overestimate จึงมีไว้ใช้ในการ exclude significant
CAD ในคนไข้ที่อยู่ในกลุ่มที่สงสัยCADไม่มาก (low to moderate likelihood of CAD)
2) Magnatic resonance imaging coronary angiography (MRI) ให้ความแม่นยาในการแปลผลในเรื่องของ
coronary vessel ค่อนข้างต่า ไม่นิยมใช้ในการประเมิน anatomy ของCAD
Invasive angiographic technique
3) Coronary artery catheterization (CAG) คือการใส่สายสวนทางหลอดเลือดแดงเพื่อฉีดสีเข้าไปที่ coronary
artery โดยตรง เป็นวิธีที่นิยมใช้มากที่สุดในผู้ปว่ ย intermediate or high likelihood of CAD และเป็น gold standard
diagnostic tool สามารถบอกค่า hemodynamic significant โดยบอกค่า coronary flow reserve ได้ อีกทั้งยังสามารถ
ทาหัตถการเช่น balloon angioplasty หรือใส่ stent ได้อีกด้วยในกรณีที่anatomy เหมาะสม
4) Positron emission tomography (PET scan) เป็นการตรวจโดยดูการ uptake และ metabolism ของ
glucose โดยใช้ Fluorine-18-labeled fluorodeoxyglucose การที่ regional accumulation ของ Glucose ยังคงมีอยูใ่ น
กล้ามเนื้อหัวใจบริเวณทีม่ ี hypocontractility แสดงว่ากล้ามเนื้อหัวใจยังมีชวิ ิตอยู่ การรักษาโดย revascularization ที่
บริเวณดังกล่าวจะสามารถทาให้บริเวณนั้นกลับมาทางานได้ดี
Treatment of Ischemic Heart disease
การรักษาโรคนี้ประกอบไปด้วย Medical treatment และการทา revascularization
Medical treatment
- Antiplatelets
- Decrease myocardial oxygen demand
Control heart rate : Beta-blocker
Control hypertension /decease after load of the ventricle : ACEI, Calcium channel blocker
- Increase myocardial oxygen supply : Nitrates
- Control risk factor : DM, Dyslipidemia , stop smoking
- Statins : หวังผลเพื่อเป็น plaque stabilizer มีประโยชน์แม้ในผูป้ ่วยที่ไม่มีไขมันในเลือดสูง
- Physical therapy and life- style modification
Myocardial Revascularization
วัตถุประสงค์หลักของการทา revascularization ในผู้ป่วย CAD คือเพื่อให้ผู้ป่วยหายจากอาการเจ็บหน้าอก และ
ลดอัตราตายเฉียบพลันจากกล้ามเนื้อหัวใจตาย (relieve symptoms and improve survival) ดังนั้นการตัดสินใจ
พิจารณาทาหัตถการเพื่อเพิม่ เลือดไปยังcoronary a. จึงขึ้นกับอาการแสดงของผู้ป่วย(symptomatic),ความรุนแรงของรอย
โรค(anatomical complexity) และ myocardial function
ปัจจุบันหัตถการในการเพิ่มเลือดไปยังเส้นเลือด coronary เพื่อ Myocardial Revascularization มี2 วิธีใหญ่ๆ
คือ
1) Percutaneous coronary intervention (PCI)
2) Coronary artery bypass grafting (CABG)
โดยการจะเลือกวิธีใดนั้น ขึ้นกับลักษณะความรุนแรงในการตีบตันของหลอดเลือด, condition และ comorbidity
ของผู้ป่วย รวมถึงความถนัดและความพร้อมของแต่ละสถาบัน ปัจจุบันด้วยเทคโนโลยีและความรูค้ วามสามารถในการทา
PCIมีการพัฒนาขึ้นเป็นอย่างมาก มีการใช้ drug eluting stent และมีการพัฒนายากลุ่ม antiplatelet ที่ดีขึ้น จึงเริ่มมีการ
ทาPCIใน difficult anatomy มากขึ้น ที่ผ่านมามีการศึกษามากมายเปรียบเทียบผลของการทา PCI Vs CABG แต่อย่างไร
ก็ตาม การศึกษาใหญ่ที่สาคัญและเป็นที่ยอมรับกันโดยทั่วไป คือ SYNTAX trial ( multicenter, prospective
randomized clinical trial ) ผลสรุปของการศึกษาแบ่งกลุ่มผู้ป่วยเป็น 2 กลุ่ม ดังนี้
- Three vessel CAD and patients with LMS disease (SYNTAX score>22) have a survival benefit and
marked reduction in the need for repeat revascularization with CABG in comparison to stents, implying that
CABG is still the treatment of choice for most of these patients
- For patients with less severe coronary artery disease ,there is no difference in survival between
CABG and stents but a lower incidence of repeat revascularization with CABG.
สรุปคือ การผ่าตัด Coronary artery bypass grafting ยังคงเป็น standard treatment of revascularization ใน
ผู้ป่วย CAD โดยเฉพาะในกลุม่ ที่มี anatomical complexityค่อนข้างสูง (SYNTAX score >22) พบว่าการทา CABG
ให้ผลดีกว่าPCIชัดเจนทัง่ ในเรื่องของ rate of re-intervention และ survival rate
ทั้งนี้ทั้งนัน้ การจะเลือกรักษาโดยวิธีใด ควรคานึงถึง “Balance short-term convenience of the less invasive PCI
procedure against the durability of the more invasive surgical approach” โดยใช้องค์ประกอบโดยรวมของผู้ป่วย
เป็นตัวช่วยพิจารณา
Indication for CABG in Asymptomatic patient and in the Chronic stable angina
1) Left main disease (≥50% stenosis)
2) Left main equivalent disease (proximal (≥70% stenosis of the proximal LAD and circumflex)
3) Double vessel disease (in the proximal LAD plus 1 other major coronary artery)
4) Triple-vessel disease (≥70% stenosis)
5) Proximal LAD disease(≥70% stenosis) combined with an LVEF of less than 50%.
Arterial conduit
1) Left internal mammary artery (LIMA) เป็นหลอดเลือดที่แตกมาจาก Left subclavian artery วางตัวอยู่ดา้ นข้างของ
sternum นิยมใช้เป็น pedicle graft เป็น arterial conduit ที่มี long-term patency ดีมากโดยเฉพาะเมื่อนามาต่อกับ
LAD เนื่องจากมีขนาดและความหนาของผนังหลอดเลือดพอเหมาะสมกับหลอดเลือดหัวใจ มีsmooth muscle ที่ค่อนข้าง
บาง จึงไม่ค่อยเกิด spasm และเมื่อเลาะจากผนังทรวงอกลงมาก็สามารถวางพาดด้านบนของหัวใจต่อกับ LAD, diagonal
artery ได้พอดี
2) Right internal mammary artery (RIMA) มีคุณสมบัตเิ ช่นเดียวกับ LIMA แต่ความยาวที่เลาะได้มักจะทาให้ต่อถึงแค่
right coronary artery ถ้าจะต่อกับ coronary artery เส้นอืน่ ก็ต้องทาเป็น free graft
3) Radial artery เป็น arterial conduit ที่เหมาะ และมี patency ที่ดี แต่ข้อเสียคือมีชั้น media ทีม่ ี smooth muscleที่
หนา ทาให้เกิดการ spasm ได้ง่าย จึงควรนาไปต่อใน coronary ที่มีการตีบมากๆ (severe stenosis) คือตีบมากกว่า 70%
ใน left coronary system และมากกว่า 90% ของ right coronary system เพื่อหลีกเลี่ยง competitive flow จาก native
vessel ก่อนจะตัดสินใจนามาใช้ควรตรวจร่างกายผู้ปว่ ยดูเสมอว่า palmar arch ของแขนข้างนั้นดีหรือไม่ โดยตรวจ
Allen’s test และไม่นิยมใช้ในกรณีที่ผู้ป่วยมี peripheral artery disease ร่วมด้วย
4) Gastroepiploic artery เหมาะสาหรับต่อกับ coronary artery ที่ inferior surface ของหัวใจ เป็น pedicle arterial
conduit ที่มีโอกาสเกิด intra-operative problem มากกว่า และมี patency ที่ด้อยกว่า radial artery มีรายงานผลของการ
ใช้ดมี ากเพียงในกลุ่มศัลยแพทย์มีความถนัดและใช้อย่างเป็นประจาเท่านั้น ไม่เป็นที่นิยมใช้กันอย่างแพร่หลาย และหาก
จะใช้ก็ควรนาไปต่อกับ severe stenotic coronary เช่นเดียวกับ radial artery
Venous conduit
Long saphenous vein (great saphenous vein; SVG) เป็น conduit ทีน่ ิยมใช้กันมากที่สุด เพราะวิธกี ารเลาะทาได้ง่าย
และรวดเร็ว เย็บต่อได้ง่าย ไม่มีปญ
ั หาเรื่อง spasm แม้ว่าจะมี long-term patency ไม่ดีเท่า arterial conduit
Other Conduits
ส่วน conduit อื่นๆ เช่น lesser saphenous vein , inferior epigastric artery ฯลฯ ไม่นิยมใช้ในการทา CABG
ใน primary revascularization แต่อาจจาเป็นต้องใช้ในกรณีที่ขาดแคลน conduit เช่น ในผู้ป่วยที่เคยทา CABG มาก่อน
หรือได้ใช้เส้นเลือดไปใช้ในการทา bypass surgery อื่นๆมาก่อนแล้ว หรือเคยทา venous stripping ไป เป็นต้น
Pre-operative assessment
การประเมินผู้ปว่ ยก่อนผ่าตัดเป็นขั้นตอนที่สาคัญมากในการดูแลผู้ป่วย เนื่องจากประวัติ หรือการตรวจพบ
บางอย่างอาจเป็นตัวสาคัญในการเปลี่ยนแปลง ตัดสินใจในการรักษา อันประกอบด้วยการซักประวัติ ตรวจร่างกายและ
ผลการ investigation ทั้งหมด นอกจากนี้ควรวิเคราะห์ข้อมูลที่ได้จากประวัตแิ ละการตรวจร่างกายว่าเข้าได้กับผลการ
investigation หรือไม่ ซึ่งเมื่อไรที่ตาม ที่ข้อมูลที่ได้มามีความขัดแย้งกัน แสดงว่ามีข้อมูลใดข้อมูลหนึ่งผิดไปจากความเป็น
จริง

ช่วงหลังผ่าตัด 6-8สัปดาห์แรก ในผู้ป่วยบางรายอาจมีอาการต่างๆต่อไปนี้
-Poor appetite
-Emotional depression
-Insomnia
-Visual deficits
-Memory and Intellectual deficits
-Loss of sexual ability
อาการต่างๆเหล่านีม้ ักหายไปได้เองภายในระยะเวลา3-6เดือน ดังนั้นควรอธิบายให้ผู้ป่วยและญาติเข้าใจเพื่อลดความ
กังวล
Operative Procedure
- Use cardiopulmonary bypass
- Aneurysm repair :
o Linear plication without aneurysmotomy
o Aneurysmectomy and repair with linear closure or repair by daron patch(Jatene’s technique)
o Endoventricular circular patch (Dor’s operation)
- Usually concomitant CABG is recommended
- In case of arrhythmia , resection of the scar should be done to remove the irritable foci

Result of surgery
Hospital mortality ภายหลังการทา VSD repair อยู่ท3ี่ 0% to 40% จะเห็นว่าแม้ทาการผ่าตัดแก้ไขก็ยังมีอัตรา
การตายค่อนข้างสูง ซึ่งสัมพันธ์กบั extent of myocardial necrosis วิธีการที่ชว่ ยลดอัตราการเสียชีวิตลงได้แก่ prompt
surgical repair, better methods of myocardial management, and more aggressive use of ventricular
assistance in the postoperative period.
Clinical presentation
- Worsening of the patient’s clinical condition
- Acute chest pain and shortness of breath
- Hypotension/cardiogenic shock
o Profound shock indicates gross MR from total rupture
o Less severe signs suggest less severe mitral regurgitation from partial rupture
Diagnosis (ควรวินิจฉัยแยกโรคกับ MR due to papillary muscle rupture, LV Dysfunction)
- Physical examination :
o New apical systolic murmur: The murmur is frequently absent in total rupture and usually
present in partial rupture.
o Apical third heart sound ( S3 gallop)
- Chest X-ray
o pulmonary edema
o heart is usually normal in size or only slightly enlarged, and the left atrium is small.
- Swan Ganz catheter
o prominent v wave in wedge position
o excluding the presence of left- right shunting from VSD
- Confirm diagnosis : echocardiography, left ventriculography
Management
- Patient stabilization
o Inotropic support
o Diuretic
o Intra-aortic balloon pump (IABP)
o Preparation for surgical intervention
Timing of operation
- Patient with cardiogenic shock : immidiate operative repair
- Patient with stable condition : operate anytime during that hospitalization
Operation
- Cardiopulmonary bypass
- Mitral valve repair or replacement
- Usually concomitant CABG is recommended Usually concomitant CABG is recommended
Clinical presentation
o Varies from asymptomatic to CHF ขึ้นกับ severity ของ MR และ ventricular function
o Presence of coronary artery disease
Diagnosis
- Physical examination :
o Apical holosystolic murmur(frequently soft and its intensity is unrelated to the severity of MR)
o May be have apical third heart sound ( S3 gallop)
- Chest X-ray : ขึ้นกับ severity ของ MR
o Pulmonary congestion, interstitial pulmonary edema, and pulmonary venous engorgement
o cardiac enlargement, including left atrial enlargement in those with moderate or severe MR
and/or severe left ventricular dysfunction
- ECG usually shows evidence of a prior MI.
- Echocardiography : hypokinesia and akinesia at the infarction site, restriction or tethering of the
leaflet (ควรแยกให้ได้กับ coexisting mitral valve disease เช่น rheumatic disease , myxomatous
degeneration, endocarditis เป็นต้น)
- Cardiac catheterization : provides information regarding the coronary arterial anatomy and
pathology
Indication for surgery
ผู้ป่วย severe ischemic MR ควรทา mitral valve repair หรือ replacement ร่วมกับการทา CABG ส่วนในกลุ่ม
ที่ variable และเป็น mild to moderate MR อาจดีขึ้นเองภายหลังการทา CABG ดังนั้น ใน mild IMR จึงไม่ควรต้องทา
การผ่าตัด mitral surgery ร่วมด้วย แต่ในผู้ป่วย moderate IMR การตัดสินใจจะขึ้นกับ individual intra-opertive
decision making
อ้างอิงจาก ESC/EACTS Guidelines on the management of valvular heartdisease 2012 ได้สรุป
indication ในการผ่าตัดไว้ดังนี้
1) Patients with severe MR undergoing CABG and LVEF >30%. (Class Ia of recommendation)
2) Patients with moderate MR undergoing CABG and LVEF >30%. (Class IIa of recommendation)
3) Symptomatic severe MR and LVEF <30%, option for revascularization, and evidence of viability.
(Class IIa of recommendation)
4) Patients with severe MR, LVEF >30%,who remain symptomatic despite optimal medical
management and have low comorbidity, when revascularization is not indicated. (Class IIb of
recommendation)
Operation
- Concomitant CABG
- Mitral valve repair with undersize annuloplasty ring or mitral valve replacement ( MVR) with chordal
preservation
Result
