AFFIDAVIT OF TWO DISINTERESTED PERSONS

KNOW ALL MEN BY THESE PRESENTS:

WE, BELLEZA LUCIDO and FARAH MATTY ECAL, of legal ages, married and
with present residence addresses at F. Calderon St., Barangay 11, Maoyod, Legazpi City,
Albay, after being duly sworn to in accordance with law, DEPOSE and STATE:

1. That we personally know ARIEL MENESES LATO, considering

that he is our godchild;

2. That we know for a fact that he is first born of Gracia Nanette

Meneses Lato and Rolando Deyto, born on November 2, 1991, because we
even attended his baptism and his birthday celebrations;

3. That perusal to his Certificate of Live Birth, per Registry No.

13039 issued by Office of the Civil Registrar of Pasig City, his name ARIEL
MENESES LATO, was inadvertently entered as ARIEL LATO and the name of
his father was left unfilled. [Hereto attached as ANNEX “A”].

4. That when he tried to obtain a copy of his Certificate of Live

Birth in the Province of Sorsogon, his name ARIEL LATO appeared to have
another entry ARIEL LATO VERCHES, also born on November 2, 1991 and
born from Sps. Gracia M. Lato and Reynaldo Verches per Registry No. 98-311,
issued by Office of the Civil Registrar of Castilla, Sorsogon; [Hereto attached
as ANNEX “B”].

5. That we certify that he was born at Poblacion Castilla, Sorsogon,

Province of Sorsogon, Philippines on November 2, 1991. That he had
been using the name ARIEL LATO VERCHES in all his official dealings
and scholastic records and transactions;

6. That we hereby attest that the person that we know with the name
ARIEL MENESES LATO and that with the ARIEL LATO VERCHES per
Registry No. 13039 and Registry No. 98-311, is ONE AND THE SAME
PERSON;

6. That we are not related by affinity or consanguinity to the

above mentioned person; and

7. That we are executing this affidavit to attest to the truth of

the foregoing facts for record and in order to apply for its Cancellation and/or
correction of Registry No. 13039.
