Wall fallen on a fork lifter.


What Happened?

On the morning of 27.09.2019, a fork lifter driver was deployed in unloading dross container in the dross
yard. At 3:00 AM during unloading dross, driver took the fork lifter back for reversal and it hit the wall
at the south side exit of the dross yard. Due to this part of the wall fell on the fork lifter, damaging its
exhaust pipe partially. No injury happened to anyone.

What we should do now?
1. Operator of the Forklift trucks to be reassessed for defensive driving skills and it must be ensured
that they undergo refresher training course successfully.
2. Immediately conduct Safety Stand down meeting and discuss about Incident in all shifts and ensure
all respective persons must be trained in HZL safety standard requirements and Safety rules.
3. High impact Wall protection like bollards or guardrails is to be provided wherever possibility of
machine interaction exists.
4. Appropriate guarding/encasing to be ensured for all other pillars in shop floor.
5. All fork lifters should have proximity sensors with audio visual alarm facility.
6. Explore the feasibility to eliminate the man machine interaction.
7. Explore the possibilities of reverse camera in all FLTs to aid backing up & alleviate rear blind spots
to avoid backup collisions
