Refer below steps:

1. Check & confirm correct tag number as per the permit to work

2. Inform to SSV/PO before commencing of the work.
3. Conduct the Safety Task analysis Risk Talk (START)
4. To check and motor status if motor abnormal sound as wo attached
5. Inform operator stop motor for remove coupling to solo run
6. Inform authorize operator to isolate the power supply of motor and authorize electrical isolator
to perform Rack Out and then apply LOTO
7. Disconnect coupling with support of ROT team.
8. Inform SSV/PO to check and ensure “there is no start command sending from DCS to MCC”
before racking in MCC
9. Inform authorize operator to remove LOTO and authorize electrical isolator to perform Rack In
and de-isolate the power supply.
10. Confirm the status of available signal feedback to DCS.
11. Inform operator to Turn ON & start the motor for solo run test to confirm motor abnormal
sound was from motor side or not. If abnormal sound from pump side, request ROT to repair.
12. If abnormal sound from the motor side, inform authorize operator stop motor to isolate power
supply & then apply LOTO.
13. Open the motor terminal box cover of motor.
14. Confirm zero voltage at the motor terminal and then discharge residual voltage.
15. Mark and then disconnect the power cable at motor terminal box.
16. Remove foundation bolts with support of ROT team
17. Use boom truck to shift motor from site to workshop
18. In unapproachable/inaccessible area, use manual hydraulic pallet stacker to shift motor to the
boom truck accessible area
19. Dismantle motor at workshop using special tools, clean bearing, rotor shaft and stator
20. Check the condition of bearing, replace with new bearing if found any abnormal sound or damage
21. Re-assemble motor, hand turn to check any further abnormal sound.
22. Use boom truck to shift motor to site
23. In unapproachable/inaccessible area, use manual hydraulic pallet stacker to shift motor from
the boom truck to the site location
24. ROT team to perform alignment of motor.
 25. Re-connect cable as per the marking & perform tightness check using torque wrench
26. Perform IR test for motor before solo run test
27. Close the terminal box cover properly and apply silicon
28. Inform operator to de-isolate for Test and perform solo run test.
29. Check and confirm the rotation direction of motor (clockwise or anti-clockwise) with Drive End
(Pump or Fan) and also witness by ROT & operation team
30. Record the current, vibration and bearing temperature values
31. Once solo run test passed, inform authorize operator to isolate power supply and then apply
LOTO
32. ROT team to re-connect coupling of the motor.
33. Inform authorize operator to de-isolate power supply and perform motor run test
34. Record the current, vibration and temperature values
35. Inform to SSV/PO after completion of task
36. Close the PTW

Ensure that personnel onsite attended the electrical safety training; Notify affected contractor and
zones department in advance of the planned working activity including marked up plot plan; START
meeting shall be conducted prior to start activity. SWMS and JSA shall be discussed in TBM & START
meeting.
