Good Day!

I am Jade B. Brigola a graduating student this coming October 2019 taking up the course BSED major in
MAPEH. I would like to formally give my explanation about the findings indicated in the registrar’s list.
Please do accept my explanation for my following discrepancies during the school year 2018-2019 1st
semester. I took EC 2 ahead before the EC 1 subject, because I thought that it is fine to enroll EC 2 while
the EC 1 subject was already closed that time, I don’t have any choice but to enroll the subject for me to
finish all my remaining subjects before I’m going to have my last practicum.

Good Day!

I apologize for the late submission of the Form 137-A as part of the requirements for the graduating
students. I sent my request personally in the registrar of the UV main campus last August 29 they said,
that the high school campus was transferred in UV Pardo and I need to submit the request in there. I
went to UV Pardo and sent the request to the high school registrar then the in-charge told me to come
back on September 3 Tuesday.

I came back on the said schedule to follow up of the releasing of the form 137-A, but the in-charge of
the registrar told me that there is a delay of processing and delivering my form 137-A, because they still
need restore the files especially those who graduated 2009 and below affected of the transferring of the
school campus.
