That, I _______________________________________, a 5th year BSPT student enrolled in Arellano

University main, and currently undergoing clinical internship (1 or 2) , hereby acknowledge that upon
taking the Re- OSCE which will be held of November 9, 2019, is bound to the consequences that upon
failing the exam again, there will be an additional 1 month rotation on top of my total number of rotations
for the school year of 2019-2020

I also acknowledge that, this additional month of rotation will also affect the status of my graduation, and
therefore abide to submit all the paper works, take the exams and quizzes that will be taken within this
additional month of rotation in order to graduate.
