AFFIDAVIT OF CONSENT

I, JESUSA L. MALANGUIS, Filipino of legal age, and residing at 102 Opalo Street, Metro Manila,
Philippines, after having been sworn according to law, hereby depose and state that.

1. I am the mother of TRISTA MAE MALANGUIS, 16 years old, born on October 19, 2003 in
Cebu City Philippines, a minor whose consent I have given to travel with her God
Mother, JULIA KHARMINNA Y. SISCAR and her family to Shanghai, China on November
21 – 26, 2019;

2. I will not be traveling and accompanying with my child, named above for the duration of
this trip’

3. I guarantee that my child, named above is in the safety of her God Mother and her
family, and I am giving my consent for the said travel;

4. I give my full consent that her God Mother and her aunt, MS. MELINDA F. YAP will
support my child, named above, financially during the said travel.
