RE: CANCELLATION OF GROUP PERSONAL ACCIDENT (GPA) PLAN

Dear Madam/Sir:

Good Day!

This is to inform your good office that I come up with the decision to cancel my GPA Insurance
effective today, June 28, 2019. I was offered the insurance plan last June 26, 2019 at Cocolife-SM Manila
Branch. I would appreciate if you would inform me through a written confirmation or through a phone
call of your confirmation on my request, hopefully within 30 days upon receiving this demand. I would
also like to request if possible, the refund of my initial payment amounting to Two Thousand Pesos (Php
2,000).

I decided to discontinue this insurance for personal reasons and sadly I am not interested anymore
on your benefits and etc.
