I, CHERYL SANDUETA VELASCO, of legal age, Filipino and resident of Velasco
Road, Turno, Dipolog City, having been duly sworn in accordance with law, hereby
depose and state:

1. That my maiden name is CHERYL SANDUETA VELASCO as stated and

registered in my original birth certificate issued by office of the Civil Registry
of Dipolog City, Zamboanga del Norte;

2. That the name appearing as “MA. CHERYL SANDUETA VELASCO”,

in my Baptismal Certificate issued by the Diocese of Dipolog Parish of Our
Laday of the Most Holy Rosary Cathedral, Dipolog City, refer to one and the
same person as herein affiant;

3. That I am executing this Affidavit to attest to the truth of the foregoing

facts and to use the same for whatever legal purpose it may serve.
