Document Checklist
We are pleased to welcome you to the BNY Mellon (India) family. As a part of our on-boarding
program we request you to submit the following documents:
Background Verification Form & Letter Of Authorization duly filled (Both forms are in the
“Forms to be submitted”) attachment
Salary slips (Last 3 Months) / Offer Letter or Increment Letter
Relieving Letter / Resignation Acceptance Letter
Clear photocopy of Pan card / Passport / Aadhar Card (All Mandatory)
Certificate and mark-sheet & Passing certificate of all educational documents
o Graduation and Post-Graduation
o 12th Class
o 10th Class
Proof of Date Of Birth – School Leaving Certificate / Birth Certificate / 10th Class Certificate
For Name Change – Proof for the name change / Marriage Certificate (If applicable)
Residential address proof: (Current and permanent address).
o Own House: Ration card/ electricity bill/ Aadhar card/ Passport copy.
o Rented House: Lease agreement or Affidavit along with electricity bill.
o Paying Guest: Affidavit from the owner along with electricity bill.
o Note: Name of the owner on the rent agreement / affidavit and the name of the owner on
the electricity bill should be the same.
