JOINT AFFIDAVIT OF COHABITATION

We, MARCELINA AREBE RUIZ, of legal age, Filipino, single and ELIASER
FAUSTINO DIOSO, of legal age, Filipino, single, and both residents of 482-B Masikap Street,
Brgy. 146 Bagong Barrio, Caloocan City, after having duly sworn to in accordance with law,
depose and state that:

1. We have been living together as husband and wife, continuously and without
interruption since 1991 or a period of more than twenty eight (28) years;

2. During our cohabitation and even until present, we have never been married and there
is no legal impediment for us to marry each other;

3. As a fruit of our cohabitation, we were blessed with three (3) children named:

4. We are executing this affidavit to attest to the truth of the foregoing circumstances and
for the purpose of being exempt from license requirement to contract marriage pursuant
to Article 34 of the Family Code of the Philippines (E.O. No. 209).
