Dear Dra. Axalan:

We are writing to inform you that we are demanding for a discount for the lab tests we ordered for our
nursing home. As of today, the orders have not been given and a status update was not provided
despite the multiple emails and requests regarding this concern.

These lab tests are needed for the patients’ record which will also serve as my basis for the future
activities I will be implementing. The tests were conducted and ordered on the 28th of April and as
stated on the contract, would be given to us by the 10th of May. It has been a week since the deadline
of the requested orders, yet our company has not received any deliveries or emails regarding the
results of our tests. We have availed the same service in the previous years and have trusted your
company to constantly exert the same efficient effort for the following orders and requests. We are
hoping this incident would not happen again.

Please do consider our demands for this situation. You can reach us through
