A) BEFORE WORK

1 Preparing of work permit Conflict with other permit Read the permit and brief the workscope during permit
coordination meeting.
Miscommunication with other Ensure that other group understand our work scope and
workgroup location where the jobs to be done.
Permit violation Ensure the working permit is approved and signed by all
resprective supervisors.
Misplace / permit lost Ensure the work permit was properly kept to prevent lost
or misplaced.
2 Worksite inspection Congested area Familiarise work area such as blocking escape route to
emrgency assembly area.
Hot area Beware of hot air and high temperature and hot surface
at working area.
Unstable structure Beware of any unstable structure and report to responsible
person if necessary.
High pressure area Keep away of any high pressure equipment or system.

A) DURING WORK

1 Do inspection Hot surface / piping / item Beware of any hot surface / piping / item. Wear suitable PPE
for safety and follow provided safety procedure.
High noise area Wear ear protection such as recommended by supervisor.

Injury Beware when use tool / equipment / chemical that high risk
to cause injury. Familiarise with basic first aid procedure.
Hot area Minimize the working time at hot area.
Wear suitable PPE when work at hot area.
Falling object Wear suitable PPE and beware with any condition that
can cause falling object especially when working at height.
Falling from height Wear safety harness and avoid any action that high risk to
cause falling.
Electrical shock cause by tool Beware when use tool / equipment that can cause electrical
/ equipment shock. Do not use any malfunction electrical tool / equipment.
Electrical shock cause by condition / act Avoid any action that can cause electrical shock. Do not use
electrical equipment when in wet.
Chemical / gas leakage Always standby with chemical / gas protection tool or
equipment when there are risk of leakage at work site.
2 Measuring the findings Wrong measurement Measure in detail area.
Always use the approved and good tool / equipment.
C) AFTER WORK

1 Housekeeping Messy working area Clean up working area. All tools / equipments must return to
their place except advised by person in charge.
Injury Ensure housekeeping was done properly. Use right tool /
equipment depend on type of housekeeping.

Setup the boundry and restrict with boundry tape and warning
sign or lamp.
Always use survey meter for safety purpose to measure the
radiation level.
Report immediately to person in charge for any accident
involving the radioactive material.
Follow right safety procedure for accident involving
radioactive material.

B) DURING WORK
NO JOB DESCRIPTION POTENTIAL HAZARDS CONTROLS
If required by client, standby person from RT worker
must be ready outside the work area in make sure he/she
can monitor the movement of non-RT worker.
Always have good communication with RT team and area
owner.
Only qualified person can involve with radiation work and
well understanding about radiation work.

C) AFTER WORK
1 Housekeeping Radiation Leakage Qualified RT worker must keep all equipment for radiation
work properly and at safe area.
Beware during dismantle the projector , winding cable and
guide tube.
Make sure survey meter(s) is in "on position" until the
houkeeping is complete -to detect any radiation leakage.
Report to person in charge the radiation work is complete and
work area is safe for non-RT worker.
Messy working area Clean up working area. All tools / equipments must return to
their place except advised by person in charge.
