Final Practicum log

For this practicum, you are not expected to have something complete for each week. You are expected

to have a plan for 40 hours to engage in the direct care experience. This Final practicum log should be

shared with your mentor and agreed upon prior to submitting to course faculty. Any changes should be

communicated to both mentor and course faculty as needed. This must be signed by your Mentor.
