Questioning to fil in pain assessment from..
One day a mother of a post operative patient to the nurse to meet the doctor to check the
condition of her child who had been treated for two days which still dit not heal with complaints
of pain
patient: assalamualaikum wr wb
Nurse: walaikumsalam wr wb, what can I dofor you mom?
patient: I want to meet with doctor witnu to check up my son
nurse: yes mom, please follow me to dr.witnu room
knock on the door………………
Nurse: excusme doc, there is patient mother she want to check up her son
Doctor: sit down please mom, what can I do for you?
patient: my has been dwo days after getting operation,he still complain that he feels sick
Doctor: yes mom, I get it what is your son name mom?
patient : his name is faisal doctor
Doctor: ok , I will check him up first
In pation room…………………
doctor: is it right your name faisal?
Faisal: yes doctor
doctor: where is the part of your body which still sick faisal?
faisal: my hand and food in the left sit sir
doctor: if I have number 0-10, 1-3 it is no problem, 4-6 it on and 7-9 is heavy and 10 is too hard,
what number do you have?
faisal: yes number 5 sir
doctor: ok after this I will give you pain medition so that your pain is less
patient : yes doc thank you
after examination and give the information from the doctor, faisal can walk and started
back healthy……….
