AFFIDAVIT OF LOSS OF OWNER’S DUPLICATE COPY OF TITLE

I, CERELINA C. LAGAN, of legal age, Filipino, married to JOEL LAGAN, and

a resident of Brgy. San Jose, Puerto Princesa City, after being sworn in accordance with
law, hereby depose and state that:

1. I am the registered owner of a parcel of land situated in Bo. of Tiniguiban, Puerto

Princesa City described as Lot 12 PSD-045316-053650 with a total area of Four
Thousand Eight Hundred Seventy-seven (4,877 sq.m.) and covered by
Transfer Certificate of Title No. 19796 issued by then Register of Deeds of
Puerto Princesa City, Atty. Luciano Roxas on February 7, 1994;

2. I diligently kept the Original Owner’s Duplicate Copy of TCT No. 19796 at the cabinet
of our house located at Cacatian Compound, Brgy. San Jose, Puerto Princesa City;

3. However, in 2005, I discovered that the Owner’s Copy of TCT No. 19796 was missing
and I could no longer find it;

4. I diligently exerted all efforts to locate the land title but to no avail, hence, for all
legal intents and purposes it is now deemed lost and beyond recovery;

5. I certify and confirm that the parcel of land covered by the said TCT No. 19796 was
never mortgaged, sold, conveyed, disposed of, or encumbered to any other person or
entity and was neither confiscated nor relinquished in favor of any government
agency or authority;

6. I attest to the truth of the foregoing statements to the best of my own personal
knowledge and based on authentic documents or records;

7. I am executing this Affidavit of Loss to support my application for a new Owner’s

Duplicate Copy of Transfer Certificate of Title No. 19796, as well as for all lawful
intents and purposes it may serve.
