corporation under the Securities and Exchange Commission;

I have been given authority to execute this Affidavit by the Corporate Secretary, Sr.

The corporation is a non-stock and non-profit educational institution and is a tax exempt
entity based on law and the Constitution of the Republic of the Philippines as follows;

Under Article XIV, Section 4(3). All revenues and assets of non-stock, non-profit
educational institutions used actually, directly, and exclusively for educational purposes shall
be exempt from taxes and duties;

I execute this affidavit to attest to the truth of the foregoing facts and for the purpose of
requesting the Philippines Statistics Authority to issue a Birth Certificate.
