2. Candidates should ensure that all columns of the Application Form are filled in correctly in their own hand in ink
or with Ball Point Pen. No correspondence will be entertained by the Commission from candidates to change any of the
entries made in the Application Form. In case of NIL information, candidates are advised to clearly write NIL or NA in the

4. The completed application form must reach the Shri Dipta Bhanu Das, Under Secretary (CSM), Union Public
Service Commission, Dholpur House, Shahjahan Road, New Delhi-110069 on or before the date specified in the forwarding
letter. The envelope containing the application should be superscribed “Application for Civil Services (Main) Examination,
2010”. Application can also be delivered at Union Public Service Commission counter by hand. The Commission will not
be responsible for the applications delivered to any other functionary of the Commission.
