Re: Alarm Trigger

DATE : January 13, 2016

This is to inform in your good office about the incident transpired around 1658H of this date, wherein
the fire alarm system here at Actimed Inc. Warehouse had synchronized ringing. Immediately pressed
to signal silence mode in control panel at Lobby by Mr. Julius Tenio Actimed Facilities on duty. Fire
alarm activated at Receiving Area/Bay D, located at second floor after conducted checking within said
location.

Advised LG Grace Evangelista, assigned Lobby Receptionist to page/inform all employees inside
Warehouse that the triggered Fire Alarm is only FALSE ALARM,

Initial finding showed that it might produce an accumulated enough amount of dust causing to the
activating alarm due to the sensitivity of smoke detectors/alarm system especially an installed beam
smoke detector at high level/near rooftop. The fire alarm system was still on silence mode due to the
ongoing renovation of Bay D & E Mezzanine Area

Highly recommend to conduct monthly cleaning/preventive maintenance to all smoke detectors and
firefighting equipment for safety and security reasons.
