(Re: Abandoning of post)

WHO : LG Seth Gobelab detailed NPM Storage (Control-4 area)

WHEN : April 15, 2012 on or about 0935H

WHERE: NPM STORAGE (Control -4 area)

WHICH: The patrol team led by Ms Ochie Balmaceda designated as Purchasing Manager, with
her subordinates IPI security Team Staff Mr Martin-bel Lilbres and safety officer Mr.
Arnold Bonifacio was conducted patrolling inside the premises.

HOW: Accordingly, the detailed guard at NPM STORAGE (control-4 area) a certain LG Seth Gobelab
caught abandoning her designated post of those patrol team.

FACTS: 00A 0935H dated 15 April 2012, when the detailed NPM Storage guard a certain LG
Seth Gobelab complaint by head of Patrol team Ms Ochie Balmaceda that upon their
arrival at the said area while conducting patrolling with the security team staff Mr
Martin-bel Libres and safety officer Mr Arnold Bonifacio of ESH, they noticed that the
aforementioned lady guard is out of place to her designated post located at NPM storage
Factory D. 00A 0937H of same date, the undersigned received information thru radio
calls from Mr. Martin Libres who has declaring that the guard on duty was missing to her
place of duty, therefore, the undersigned immediately proceeded to the stated area to
conduct initial investigation why our guard on duty

Gobelab complaint by head of Patrol team Ms Ochie Balmaceda that upon their arrival at
the said area while conducting patrolling with the security team staff Mr Martin-bel
Libres and safety officer Mr Arnold Bonifacio of ESH, they noticed that the
aforementioned lady guard is out of place to her designated post located at NPM storage

calls from Mr. Martin Libres who has declaring that the guard on duty was missing to her
place of duty, therefore, the undersigned immediately proceeded to the stated area to

conduct initial investigation why our guard on duty committed violation for
abandonment of her post.

Investigation revealed that based on her statement to the undersigned, she admit her
violation but she reasoned out that during that time, she feel so much severe urinating
as well as the saturated stain of her napkin that needs to replace due to she has monthly
period, if ever she has opted to go to comfort room even without assist reliever and as
she explained even the assist reliever is progress for her to able to assist she cant able to
awaiting because the liquid waste product on her abdomen is about to release.

ACTION TAKEN: The undersigned internal duty Officer conducted investigation and accomplished
Information report regarding her infraction
